What Is Epilepsy? Types of Seizures and Treatment Approaches

Epilepsy is a common neurological condition globally. Rather than a single disease, epilepsy is a group of disorders characterized by recurrent seizures that occur without an immediate, identifiable trigger. These seizures result from temporary, abnormal electrical activity in the brain, which can affect a person’s movement, speech, or sensory perception.

We believe that patient education is a vital component of neurological care. By understanding the complexities of this condition, patients and their families can work closely with healthcare providers to navigate their management journey with greater clarity.

The Different Types of Seizures

Seizures manifest in various ways depending on where the electrical activity originates in the brain. They are not always characterized by physical convulsions.

  • Focal Onset Seizures: These begin in a specific area or network of cells on one side of the brain. A person may remain fully aware but experience unusual sensations, such as a “rising” feeling in the stomach or phantom smells. Alternatively, they may experience impaired awareness, appearing dazed or engaging in repetitive movements like lip-smacking.
  • Generalized Onset Seizures: These involve both sides of the brain from the start. This category includes absence seizures (brief lapses in consciousness, common in children), tonic-clonic seizures (muscle stiffening followed by rhythmic jerking), and myoclonic seizures (brief, shock-like muscle jerks).
  • Unknown Onset Seizures: If the start of a seizure is not witnessed or cannot be identified, it is classified as “unknown” until further clinical monitoring provides more data.

Diagnostic and Management Approaches

The management of epilepsy has evolved significantly. Today, healthcare providers prioritize personalized care plans tailored to the specific needs of each patient.

Accurate Diagnosis

The first step involves a comprehensive evaluation. This typically includes:

  1. Medical History: A detailed review of symptoms and events.
  2. Electroencephalogram (EEG): A test to record brain wave patterns.
  3. Neuroimaging (MRI): High-resolution scans to identify potential structural causes.
  4. Extended Video-EEG Monitoring: Used in cases where standard tests are inconclusive to capture and analyze events in real-time.

Management Strategies

Most individuals with epilepsy manage their symptoms effectively through Anti-Seizure Medications (ASMs). Recent developments in pharmacology have led to the availability of medications designed to minimize side effects while maintaining efficacy.

For patients who do not respond optimally to traditional medication, other clinical options may be considered:

  • Neuromodulation: Uses electrical pulses to help regulate brain activity.
  • Dietary Therapies: Specific medically-supervised diets.
  • Surgical Evaluation: In specific cases of focal epilepsy, surgery may be a viable option to address the seizure focus.

Dedicated Neurological Care

Living with epilepsy involves managing both physical symptoms and overall quality of life. Our clinic is led by Dr. Zhao Yi Jing, a Singapore-trained and certified neurologist with clinical experience at major institutions, including Singapore General Hospital (SGH) and the National Neuroscience Institute (NNI).

Our practice combines clinical experience with a patient-centered approach. We provide a range of neurology services, including advanced EEG monitoring for diagnostic clarity, the management of complex conditions such as stroke and Parkinson’s disease, and specialized treatments for chronic headaches. We aim to provide patients with evidence-based management options by staying informed on current clinical research and neurological therapies.
